alright, got a 99 k2500 4x4 auto 5.7. About a week ago it began acting strange. it would run reg temps (190) and randomly spike to 250 or so, would rapidly (after4-5 seconds) return to reg temps. Would not boil over. plenty of coolant, plenty of airflow, everything "looked" ok. replaced thermostat, no change. Does not appear to be load sensitive, ran it with the heater on lo, dial set midway through temp range, ran fine. WTF? fan clutch appears fine...

My only thought is that if the radiator hose is old they can collapse and cut off the coolant flow, and having the heater on would create another path for the coolant to get to the engine. I can't explain it returning back to normal after 4-5 seconds though...

you either got a bad gauge or sending unit

COuld be a bad ground to or from the block, sends all kind of wierd signals.
i'll check out the hoses. the engine does get HOT its not just the gauge i don't think, as the motor "Pings"
