codes 12 14 21 31 42

I had the exaust redone but I put a new O2 sensor in(after the cat).. Still getting these codes. After the rig heats up it starts to BOG and load up. Like it is getting to much fuel..... Thinking of replacing the MAP sensor next.. any ideas?

12-battery was disconneced, 14-MAP sensor voltage, 21-o2sensor, 31-Evap leak, 42-fuel sending unit These are what the codes mean. Are all conections tight?
