Back in the 80`s when Smith & Wesson introduced the 629 series, I had a brand new 6" do the same thing on the 5th shot down the tube. I promtly returned the gun to where I bought it and they gave me another to replace it. Took it out and got 25 rounds through it and the barrel unscrewed itself from the frame. I returned it also and haven`t bought another S&W to this day.
