I am a FORD man all the way but have a buddy that is trying to part out a truck and needs to know what trans/T-case is in it.
It is an 89 K1500 with a 4.3, manual 5 speed. How many options are there in those or is there only one and what is it?

There should be a metal tag on the back of the case,
most likely a new process 208 or 241.

The Tcase is a 241C drivers drop aluminum chaindrive slipyoke.
The tranny is an 88mm/LM5?/getrag light duty piece of shit, perfect for a V6 but available behind V8's. Integral bellhousing, external hydraulic clutch slave. Nonrebuildable by mortals.

Looking around i heard they were NV3500(or possibly the 5LM60) tranny with the 208 transfer.
not sure what options you could want to know though??
