what to do now?

A week or so back I mailed off a form 4 for my YHM suppressor and a form 1 for a .22 suppressor I want to build. Well today I check the mail and there is a letter there with the check for my form 4, but nothing else So I have no idea what happened to the rest of the paperwork, and I'm waiting for a response from my class 3 dealer to figure out what to do. This makes me worry about my form 1, even though its in a separate envelope going to a different address.
