Alot of times the roller bearings in the lifter will come apart, and for some stupid reason the screen in the oil pump pickup is big enough to fit the needle bearings through. So when you get it apart make sure you check the oil pump to see if any metal went through it.
You may be able to do a partial cab lift and get it out, thats how dealers are doing high pressure pumps and stuff. You just pull the front bolts behind the bumper and loosen the rest of the cab bolts, and then jack the front up and slip some wood underneath it, that usually gives you quite a bit of room.
