I used BTB enterprises plate. I think it is like $60 but when I needed it making one would have been really hard. A couple of months ago Toyota trails did a Writeup on the conversion and had the specifications too. Their online writeup does not appear to have the specs though.

the hard part is cutting 3/8's plate and getting the holes to line up.

BTW make sure you box the other side too!!!!!!

i have the metal taht i can use to make it, i just need some tubing for the spacers off the plate..if no one else responds, i could try to make you one, all you have to do is drill where the holes go and weld on spacers where needed...

i've got a brand new one sittin' in the garage. call me and i'll give you the dimensions or i can e-mail you a pic and the dimensions. they're easy to cut.
mine is not for sale; it's a template to make more.f&*# BTB and their copyright(if they have one), as if they came up with the plate or any of the other stuff they're peddling!

dimensions:
from top (longest part) in clockwise fashion:
10 inches, 2.5, 3.5, .75, .75, 3.0, 4.25 and 4.
these are pretty close and will definetly get you there.
the spacers from top left clockwise are .25, .75, .75 and the 4th space is optional if you have a 4 bolt pump.
get it!
