I just swapped in a new starter yesterday.
It was in a 22RE but I had the same issue. I used a 6" extension and went over the top of the starter, it stuck out just past the end of the starter and I was able to put my ratchet on. To put it back on I layed under my rig with my feet towards the drivers side and this position allowed me to bend/reach my hand in and start that same bolt back on, then back to the ratchet with extension. It isn't to bad once you figure out how to get to the bolt. Hope this helps...

I just reach over the fender with a box end wrench, honestly the toyota trucks have the easies starters to remove that I have EVER seen. Hope you guys never have to mess with an El Camino <IMG SRC="smilies/eek.gif" border="0">

sometimes you can get enough extensions so the ratchet is outside of the truck, just gotta get creative..

18" extension AND a tape-wrapped universal. Must all be Snap-on of course.... But seriously DO NOT drop that bolt in the starter hole next to the clutch or you are in for a whole lotta hurtin'....

Not sure about your year of truck but my 89 4runner has a access panel on the inner fender that you can take off to get to the starter and fuel filter easier..... You just do everything from the wheel well...
